Why Mercury-Safe Removal Matters

Dental amalgam fillings contain approximately 50% mercury by weight. While the fillings are stable when intact, the removal process — if not performed carefully — can release significant amounts of mercury vapour and particulate matter.

Mercury is a recognised neurotoxin. The IAOMT's SMART protocol exists to minimise exposure during removal — for the patient, the dental team, and the environment.

At SHDC, every amalgam removal is performed using the full SMART protocol. We were among the first practices in Sydney to adopt this standard, and it remains central to our clinical philosophy.

The SMART Protocol Includes

  • Rubber dam isolation
  • High-volume evacuation
  • Sectioning technique (chunk removal)
  • Activated charcoal rinse
  • Protective gowns & masks
  • Air filtration in surgery
  • Biological support guidance

Common Questions

Is amalgam removal safe?

When performed using the SMART protocol, amalgam removal is significantly safer than standard removal. The protocol uses rubber dams, high-volume evacuation, and sectioning techniques to minimise mercury vapour exposure.

What is the IAOMT SMART protocol?

SMART stands for Safe Mercury Amalgam Removal Technique. It is a set of evidence-based procedures developed by the International Academy of Oral Medicine and Toxicology to protect patients, practitioners, and the environment during amalgam removal.

How many appointments does it take?

Most patients require between 2–4 appointments depending on the number of amalgam fillings. We recommend removing one quadrant at a time to minimise systemic exposure.

What replaces the amalgam?

We replace amalgam with high-quality, biocompatible composite resin or ceramic restorations — materials that are mercury-free, BPA-free, and matched to your natural tooth colour.
